How To Become A Client or Patient - 4 Steps

Step 1.  Please print, read and sign the following  forms when indicated. Bring all these forms to your appointment. Contact us if you have any questions.

Psychological Services:

• Office Policies and Informed Consent Acknowledgement (PRINT & SIGN)

Forensic and Legal Support Services:

• Forensic Office Policies and Informed Consent Acknowledgement (PRINT & SIGN)

Step 3.  Please complete the following form and bring to your first appointment. 

Contract for Services (PRINT & Sign)

Information Release forms (PRINT & BRING)

Step 4.  If asked, please complete our Background and Information form. Please complete this form by hand or on-line. Please bring this to your first appointment. 

Adult: 

• Client/Patient Information Form. For adults or couples

Child/Youth:

• Client/Patient Information Form. For children and youth.
   
• StepOne for Parents. This will take you to another web site where you need to complete this online screening.

Policy.  We can seek re-imbursement from their insurance in most cases. We can bill insurance companies directly for  you. In some case we will negotiate a fee with you.
