We are glad to answer any questions regarding our service on the phone or during your first appointment.

Step 2. Please print, read and sign the following three forms. Please mail, or bring it to your first appointment.

Step 3.  If asked, please complete our Background and Information form on-line. Please complete this form by hand or on-line. You may submit on-line, mail, or bring it to your first appointment. 

Adult:  Client/Patient Information Form . For adults or couples

Child/Youth: Client/Patient Information Form. For children and youth.

Step 4.  If asked, please read the Fees and Payment.  This will outline our fee and services. We do not charge for the first appointment if we decide not to work together.

Adult:  Release of Information

Child/Youth:  Release Of Information (under 18 years old.)

Policy.  We help our patients seek re-imbursement from their insurance. We do not bill insurance companies directly for  you. Our fees are established to provide services and to financially support our non-profit information web sites. We do not offer reduced fees or a sliding scale. Payment for services are by check or credit card.
