Call us at (541) 388-5660 (local in Bend, Oregon) or E-mail us to see if we can help. Step 2. Please print, read and sign our "Information and Informed Consent" form. Please mail, or bring it to your first appointment. Step 3. (Youth) If asked, please complete our Youth Background and Information. Please complete this form by hand or on-line. You may submit on-line, mail, or bring it to your first appointment.
Step 4. If asked, please read the Fees and Payment. This will outline our fee and services and allow you to pay. Step 5. If asked, please print and complete the Client and Patient Contract. You may mail, or bring it to your first appointment. Step 6. If asked, please print and complete the following "Release of Information".
Policy. We help our patients seek re-imbursement from their insurance. We do not bill insurance companies directly for you. Our fees are established to provide services and to financially support our non-profit information web sites. We do not offer reduced fees or a sliding scale.
